The defective and potentially dangerous USB power adapter was packaged with each iPhone 3G sold in US, Japan, Canada, Mexico and quite a lot of other Latin American countries. Apple has revealed the Apple Ultra compact USB Power Adapter swap over program for iPhone 3G sold in select countries. Rumor has it that, the adapter's metal prongs can break- off and hang about in a power outlet, creating jeopardy of electric fright. There were even a few reports of cut off blades involving an extremely small percentage of the adapters sold.
